   Sekiranya pembayaran adalah secara potongan Vot Pusat Tanggungjawab, peserta hendaklah mengisi borang potongan vot jabatan yang disediakan. Borang yang telah lengkap diisi hendaklah disahkan oleh ketua jabatan dan perlu dimuatnaik di ruangan "Bukti Pengesahan Pembayaran".
